Required Documents

All new students must have a physician complete a physical exam documented on a DH3040 form.

Authorization form for the administration of medication during school hours 

Lower School:

8:00 am - 2:15 pm

Upper School:

8:00 am - 2:45 pm

Uniforms

Only short-sleeve shirts (FCA Spirit Fridays) and outerwear (daily wear) are approved to wear to school.
